1. Wejdź na stronę azs.pl/legitymacja-isic i zjedź niżej, aż do nagłówka Co chcesz zrobić?
2. Jeżeli legitymację członkowską wyrabiasz po raz pierwszy - wybierz przycisk Wypełnij deklarację na rok akademicki 2026/2027.
3. Wypełnij deklarację członkowską, zgodnie z poniższą instrukcją i opisami przy polach.
4. Zadbaj o dobrą jakość dołączonego zdjęcia.
5. Wybierz pakiet - Standard / Komfort / Sport, w zależności od preferencji. Warianty różnią się ceną składki i tym samym kwotą ubezpieczenia NNW. Jest to opłata jednorazowa na dany rok akademicki (01.10.2026 r. - 30.09.2027 r.).
   
   W roku akademickim 2026/2027 składka dla odpowiedniego pakietu wynosi:
   
   • Standard NNW 8.000 zł - 180,00 zł
   • Komfort NNW 25.000 zł - 230,00 zł
   • Sport NNW 65.000 zł - 1.180,00 zł.
     
     WAŻNE - w przypadku wyboru złego wariantu, patrz punkt UWAGI na dole!¹
     
     
6. Jeżeli chcesz, do 30 listopada 2026 r. możesz wnioskować o otrzymanie legitymacji również w wersji plastikowej. W tym celu, w kolumnie Typ legitymacji członkowskiej AZS, zaznacz pole elektroniczna i fizyczna. Podstawową (i wystarczającą do trenowania oraz startu w AMP) jest legitymacja elektroniczna. Opłata za legitymację plastikową wynosi 35,00 zł. Następnie, legitymacja zostanie wysłana na adres podany w deklaracji.²
7. W polu Rodzaj legitymacji członkowskiej AZS wybierz AZS-ISIC, jeżeli jesteś studentem, bądź AZS-ITIC, jeżeli jesteś pracownikiem. Dołącz zdjęcie dokumentu potwierdzającego Twój status (przód i tył).³
8. W polu Uczelnia wybierz Uniwersytet Medyczny w Lublinie, poniżej wybierz Województwo Lubelskie, a po prawej wybierz Klub AZS - AZS UM Lublin. Następnie wybierz sekcję, do której chcesz dołączyć i po prawej wpisz datę, kiedy po raz pierwszy wziąłeś udział w treningu.
9. Po zaznaczeniu zgód, sprawdź poprawność danych i wybierz przycisk Potwierdzam, że wpisane dane są prawidłowe i nie ma w nich błędów (pokazany obok/poniżej).
10. Poczekaj na akceptację wniosku przez Klub. Gdy to nastąpi, dostaniesz wiadomość e-mail, w której zostaniesz przekierowany na stronę Planeta AZS.
11. Zaloguj się do systemu Planeta AZS, zgodnie z instrukcją w mailu.
12. Jeżeli widzisz napis Panel członka, zjedź na dół strony, aż do napisu Historia transakcji i wybierz Opłać składkę. Można ją opłacić wyłącznie elektronicznie.
13. Członkostwo odnawia się od razu po opłaceniu składki, natomiast ubezpieczenie ważne jest od kolejnej doby (jednakże nie wcześniej, niż od 1 października 2026 r.) do 30 września 2027 r.

UWAGI:

• ¹ JEŻELI POMYLIŁAŚ/POMYLIŁEŚ PAKIET (np. wybrałaś/wybrałeś Sport, a chciałaś/chciałeś Komfort) - skontaktuj się bezzwłocznie z Zarządem Klubu (azs@umlub.edu.pl). Zarząd anuluje Twój wniosek, który będziesz musiał wypełnić od nowa. Nie ma możliwości zmiany wybranego pakietu (w domyśle - ceny składki) po kliknięciu przycisku Potwierdzam, że wpisane dane są prawidłowe i nie ma w nich błędów.
• ² Nieuiszczenie opłaty za plastikową legitymację do 7 grudnia 2026 r. spowoduje anulowanie wniosku o jej wyrobienie, a kwota 35,00 zł nie podlega zwrotowi.
• ³ UWAGA - aplikacja mObywatel nie daje możliwości zrobienia zrzutu ekranu, więc jeśli chcesz załączyć mLegitymację, to zrób zdjęcie ekranu innym telefonem. Jeżeli jesteś studentem I roku, zamów mLegitymację przez USOS (zakładka DLA STUDENTÓW, trwa to ok. 1 minutę).

Zachęcamy do pobrania aplikacji ISIC z App Store oraz Google Play i korzystania z obecnych tam zniżek i promocji.

3. Zaloguj się do portalu Planeta AZS. Podaj e-mail i hasło, których używałeś rok (lub więcej lat) temu. Jeżeli nie pamiętasz hasła, skorzystaj z opcji jego odzyskiwania - Ustaw nowe hasło.
4. Rozwiń MENU i wybierz Przedłużenie ważności legitymacji członkowskiej AZS 2026/2027.
5. Potwierdź poprawność swoich danych bądź jeśli Twoje dane nie zgadzają się, przejdź na formularz Zmiana danych osobowych.
6. Jeżeli posiadasz orzeczenie o niepełnosprawności, uzupełnij potrzebne informacje.

Every AZS member is required to hold a valid AZS membership card. The membership fee includes personal accident insurance (NNW), with the coverage amount depending on the selected package (Standard / Komfort / Sport), as well as membership fees allocated to the AZS National Board, the AZS Regional Organization of the Lublin Voivodeship, and the University Sports Association Academic Club at the Medical University of Lublin. It is not possible to select a membership package without insurance. Membership in the Club is mandatory for participation in Club training sessions and for representing the Club in competitions such as the Lublin Voivodeship Academic Championships (AMWL), Polish Academic Championships of Medical Universities (AMPUM), and Polish Academic Championships (AMP).

Before applying for your AZS membership card for the first time or renewing your existing card, PLEASE READ THE INSTRUCTIONS BELOW. If you have any questions or encounter any problems, please contact the Coordinator of the sports section you wish to join or the Club Board at azs@umlub.edu.pl.

Select the appropriate application process for your AZS membership card by visiting azs.pl/legitymacja-isic and choosing the relevant option, according to the instructions provided below.

This is the page you will see after clicking the link above. Select the option on the left or right, depending on whether you are completing the membership declaration for the first time or renewing it.

1. Go to azs.pl/legitymacja-isic and scroll down until you reach the heading Co chcesz zrobić? (“What would you like to do?”).
2. If you are applying for an AZS membership card for the first time, select the button Wypełnij deklarację na rok akademicki 2026/2027 (“Complete the membership declaration for the 2026/2027 academic year”).
3. Complete the membership declaration in accordance with the instructions below and the descriptions provided next to each field.
4. Note the section Obywatelstwo polskie ("Polish nationality") and select Nie ("No"). There will appear two boxes - Kraj pochodzenia ("Country of origin") and Seria i nr paszportu ("Passport series and No.")
5. Make sure that the attached photo is of good quality.
6. Select one of the available packages — Standard / Komfort / Sport — according to your preferences. The packages differ in membership fee and, accordingly, in the amount of personal accident insurance (PAI = NNW) coverage. The fee is a one-time payment for the entire academic year (1 October 2026 - 30 September 2027).
   
   For the 2026/2027 academic year, the fees are:
   • Standard NNW PLN 8,000.00 – PLN 180.00
   • Komfort NNW PLN 25,000.00 – PLN 230.00
   • Sport NNW PLN 65,000.00 – PLN 1,180.00
     
     IMPORTANT — if you select the wrong package, please see the NOTES section below.¹
     
     
7. If you wish, until 30 November 2026, you may also request a physical membership card. To do so, select the option elektroniczna i fizyczna (“electronic and physical membership card”). The electronic membership card is the standard and sufficient option for participating in training sessions and competing in the Polish Academic Championships (AMP). The fee for the plastic card is PLN 35.00. The cards will then be sent to the address provided in the application, in the order in which membership fees are paid.²
8. In the Rodzaj legitymacji członkowskiej AZS (“Type of AZS membership card”) field, select AZS-ISIC if you are a student or AZS-ITIC if you are a university employee. Attach a photo of a document confirming your status (front and back).
9. In the Uczelnia (“University”) field, select Uniwersytet Medyczny w Lublinie. Then, below, at Województwo, w którym działa Klub AZS ("Voivodeship in which Club operates"), select Województwo Lubelskie, and on the right select Klub AZS – AZS UM Lublin. Next, select the sports section you wish to join and, in the field on the right, enter the date on which you first participated in a training session.
10. After selecting the required consents, check that all your personal details are correct and select the button Potwierdzam, że wpisane dane są prawidłowe i nie ma w nich błędów (“I confirm that the information provided is correct and contains no errors”), shown next to/below the relevant section.
11. Wait for the Club to approve your application. Once it has been approved, you will receive an e-mail directing you to the Planeta AZS website.
12. Log in to the Planeta AZS system according to the instructions provided in the e-mail.
13. If you see Panel członka (“Member Panel”), scroll down to Historia transakcji (“Transaction History”) and select Opłać składkę (“Pay membership fee”). The fee can be paid exclusively online.
14. Your membership becomes active immediately after the membership fee is paid. The insurance becomes effective from the following day, but not earlier than 1 October 2026, and remains valid until 30 September 2027.

NOTES:

• ¹ IF YOU SELECTED THE WRONG PACKAGE (e.g. you selected Sport but intended to select Komfort), contact the Club Board immediately at azs@umlub.edu.pl. The Board will cancel your application, and you will have to complete a new one. It is not possible to change the selected package (and therefore the membership fee) after clicking the button Potwierdzam, że wpisane dane są prawidłowe i nie ma w nich błędów (“I confirm that the information provided is correct and contains no errors”).
• ² Failure to pay the PLN 35.00 fee for the plastic membership card by 7 December 2026 will result in the cancellation of your request for a plastic card. The PLN 35.00 fee is non-refundable.

3. Log in to the Planeta AZS portal using the e-mail address and password you used one or more years ago. If you do not remember your password, use the password recovery option - Ustaw nowe hasło ("Set new password").
4. Open the MENU and select Przedłużenie ważności legitymacji członkowskiej AZS 2026/2027 (“Renewal of AZS Membership Card 2026/2027”).
5. Confirm that your personal details are correct. If any of your details are incorrect, go to the Zmiana danych osobowych (“Change of personal details”) form and update them.
6. If you have a disability certificate, please provide the required information.
7. Select one of the available packages — Standard / Komfort / Sport — according to your preferences. The packages differ in membership fee and, accordingly, in the amount of personal accident insurance (PAI = NNW) coverage.
